During the application process you will be asked to provide information and documents. Use this checklist to determine what documents are applicable to you and forward to your Loan Officer as soon as possible. Additional items not on this list may need to be provided to complete your loan. If other documentation is required, you will be contacted by your loan officer or other support personnel.


Government issued photo ID to verify your identity and legal residency:

  • Current Driver’s license or State-issued ID or U.S. military ID as applicable
  • Copy of Social Security card.
  • Copy of Permanent Resident Alien Card if applicable
  • US Passport

Residence History

The street address for every residence you have lived in during the past two years. And, If you are currently renting, provide:

  • Landlord’s name , address and phone number

If you own your current residence, provide:

  • Current Mortgage billing statement
  • Evidence of hazard insurance (i.e. Declaration Page)
  • HOA Billing Statement - if applicable

Credit History

If you have experienced derogatory credit events such as Bankruptcy, Short Sales, Deed-In Lieu of Foreclosure or Foreclosure over the last 7 years, provide:

  • Signed letter of explanation.
  • Bankruptcy Petition with all schedules and Discharge of Debtor
  • Address of properties with prior short sale, deed in lieu of foreclosure or foreclosure.
  • Final closing statement for any properties sold as a short sale


Certain “life events” require additional documentation to determine if you have recurring monthly obligations that must be considered in loan qualification. Provide any of the documentation as applicable:

  • Final Divorce Decree signed by the court and Marital Settlement Agreement for all prior marriages.
  • Child Support Orders for all child support obligations
  • Court ordered payments for judgments and liens
  • Payment arrangement documentation for any State or Federal tax liens or other payments

Veteran’s Administration Loan Applicants

If you are applying for a Veteran’s Administration Loan, you need to provide one of the following documents:

  • Military discharge papers (Form DD214)
  • In Service Letter, if currently active

Sales Contract

  • Completed and signed sales contract for the property you are purchasing

Real Estate Information

You need to provide the following information about any real estate you own:

  • Address of property
  • Describe use of property as primary, second home or investment
  • Market value, estimate
  • Loan information (i.e. current balance and monthly payment statements)
  • Copies of Leases or Rental Agreements on rental properties owned
  • Property Tax payments
  • Home Owners Association Dues (HOA), if any
  • Copy of Hazard Insurance policy declaration page to verify cost

Employment History

You need to provide the following information for every employer you have worked for during the past two years:

  • Employer's name
  • Employer's address
  • Employer's phone number
  • Signed letter of explanation for employment gaps exceeding one month

Income Information

You will need to provide the following information about your income to complete your application:

  • Gross monthly salary or other income
  • Year-to-date pay stubs covering the most recent 30 days
  • W2s and 1099’s, if any for the past two years
  • Personal tax returns for the past two years.
  • Award or Benefit letter for Social Security, Retirement/Pension or Disability pay
  • Proof of other sources of income covering a two year history

If you’re self-employed, you will need to provide:

  • Personal tax returns for the past two years (Form 1040)
  • Business or partnership tax returns for the past two years (Form 1065 or Form 1120, plus K-1)
  • Current profit and loss statement
  • Current balance sheet

Deposit, Retirement and Equities Account Information

For all checking, savings, IRA, 401(k) or other retirement program, stock and mutual fund accounts, you will need to provide:

  • Statements for the past two months, include all pages

Personal Property Information

You need to provide the estimated value of personal property on your application including:

  • Vehicles (include year, make and model)
  • Household & personal property, including tools, furniture, clothing, etc
  • Boats
  • Campers
